چکیده

Patients with cerebellar infarction should not be neglected because they can experience sudden clinical deterioration from cerebellar swelling. Compared with malignant middle cerebral artery infarctions, malignant cerebellar infarctions can be more urgent because of the smaller space of the posterior fossa, consecutive brain stem compression, and obstructive hydrocephalus. In these situations, suboccipital decompressive craniectomy (SDC) is recommended as a lifesaving therapy although no randomized controlled trials on this therapy have been conducted. Furthermore, no evidence is available on the benefits of preventive SDC for patients with cerebellar infarction, whereas the benefits of early preventive decompressive hemicraniectomy for malignant middle cerebral artery infarction are supported by randomized controlled trials. However, a subset of patients might be expected to benefit in terms of improved survival and functional outcomes from preventive SDC. Determining how to select patients who are good candidates for preventive SDC would be useful. Thus, the purpose of this matched case–control study was to investigate whether preventive SDC was associated with good clinical outcome in patients with cerebellar infarction and to evaluate its predisposing factors.

Treatment of cerebellar infarction by decompressive suboccipital craniectomy.

BACKGROUND AND PURPOSE We present an anecdotal series of 11 patients without past history of stroke with progressive neurological deterioration while on medical therapy for large cerebellar infarctions. Clinical signs of brain stem compression developed in these patients. Computerized tomography of the head confirmed mass effect from brain edema. It was the clinical judgment of the neurologists...

Decompressive Craniectomy in Posterior Fossa Ischemic Stroke

Ischemic damage produced in the posterior cerebral territory causes significant morbidity and urgently must be considered if the patient need a surgical attitude. Surgical decompression by suboccipital craniectomy seams to be effective to treat secondary edema due to cerebellar damage or in posterior fossa, when medical treatment is not able to control side effects. We report a clinical case of...

Long-term outcome after suboccipital decompressive craniectomy for malignant cerebellar infarction.

BACKGROUND AND PURPOSE Suboccipital decompressive craniectomy (SDC) is a life-saving intervention for patients with malignant cerebellar infarction. However, long-term outcome has not been systematically analyzed. METHODS In this monocentric retrospective study we analyzed mortality, long-term functional outcome, and quality of life of all consecutive patients that were treated by SDC for mal...

Intraoperative Visualization of Bilateral Thrombosis in the Posterior Inferior Cerebellar Artery Apparent in the Telovelomedullary Segment

Unilateral posterior inferior cerebellar artery (PICA) thrombosis is frequent. However, bilateral PICA thrombosis is rare. Herein we report about an intraoperative visualization of a bilateral thrombosis of the telovelomedullary segment of the PICA. A 74-year-old woman was admitted to our department on day two of a bilateral PICA thrombosis with developing cerebellar infarction. Her Glasgow Com...

Long-term outcome after surgical treatment for space-occupying cerebellar infarction: experience in 56 patients.

BACKGROUND AND PURPOSE Surgical management of space-occupying cerebellar infarction is still controversial. Data on long-term outcome are lacking. The objective of this study was (1) to evaluate outcome after at least 3 years poststroke in patients with space-occupying cerebellar infarction treated by ventriculostomy/extraventricular drainage (EVD) or suboccipital decompressive craniectomy (SDC...
